To align the edges of the shirt cuff and sleeve placket, try basting the cuff to the placket along the edge perpendicular to the seam you are about to sew. This keeps the cuff in place when you sew the cuff to the sleeve, whether by machine, by hand, or for basting.
On the inside of the cuff, make sure when you baste that you catch the seam allowance and sew it flat so that you don't get any weird bumps.
Voila. Nicely aligned cuffs!
This works for collar stands, too.
